Intake Coordinator - Home Health (Saturday/Sunday,Days)
Job DescriptionResponsible for accepting telephone referrals and coordinating initial in-home assessments for patients that are referred to Home Health.
This individual will be the first point of contact for the agencies and will be responsible for communicating and coordinating patient care with hospitals, physician offices, and other referral sources.
EducationHigh School Diploma plus 1 year of vocational schoolExperienceOne year of related experience.
Requires a working knowledge of standard practices and procedures.
Licenses Certifications Driver's License Vehicle InsuranceQualifications Good organizational skills are required.
Good verbal and written communication skills are required.
Must have proficient computer skills.
Experience with Maestro and Meditech clinical application systems preferred.
Prefer a minimum of two years of experience working in healthcare with knowledge of in-take and patient scheduling and insurance verification.
